Survey Wizard supports an unlimited number of surveys, each with an unlimited number of questions.
For each question in a survey, you specify how the answers will be presented to visitors: in a select list, as radio buttons, as check boxes, or as a text input field.
Each question can have up to 10 predefined answer choices. When a user completes a survey, their responses are tabulated and added to the database.
Survey Wizard's administrative interface lets you easily add surveys, disable surveys so that no one can take them anymore, and view the results of any survey. Optionally, you can also allow your visitors to view survey results as well.
